A 7-year-old Nigerian child is treated at a regional medical center for a facial tumor. The mass is located within the mandible itself. Which of the following is true regarding this patient's disease?
AMost of the cases in HIV-positive patients are associated with Epstein-Barr virus infection.
BThis disease is caused by up-regulation of the c-myc oncogene
CTumor lysis syndrome is uncommon in this disease
DThe disease is usually unresponsive to chemotherapy
Show Answer & Explanation ▾
✓ Correct Answer: B. This disease is caused by up-regulation of the c-myc oncogene
Translocation between the c-myc gene and immunoglobulin genes is the defining feature of a Burkitt lymphoma. In 80% of cases the translocation, t(8;14), is between c-myc and the immunoglobulin heavy chain gene. The remainder of the cases involve translocation of c-myc with immunoglobulin light chain genes. Epstein-Barr virus is involved in almost all cases of African (endemic) Burkitt lymphoma, but fewer than 30% of sporadic or immunodeficiency related cases. This malignancy has one of the highest turnover rates known, making it extremely fast growing but also very chemo sensitive. Because of this fact, the risk of tumor lysis syndrome is very high. The neoplastic cells in Burkitt lymphoma do not express terminal deoxynucleotidyl transferase (TdT), distinguishing it from acute lymphoblastic leukemia.
