About Myelodysplastic Syndrome, Myeloproliferative Neoplasms and Histiocytosis MCQs

Myelodysplastic Syndrome, Myeloproliferative Neoplasms and Histiocytosis MCQs range across BCR-ABL transcripts in chronic myeloid leukaemia, polycythaemia vera with aquagenic pruritus, and Langerhans cell histiocytosis with its Birbeck granules. These are high-yield NEET PG questions for FMGE, INI-CET and NEET SS.

Sample Myelodysplastic Syndrome, Myeloproliferative Neoplasms and Histiocytosis MCQs with answers

Sample MCQ #1

True about BCR-ABL 'traits' are all except:

AP190 has an indolent course
BP190 is a bad prognostic factor
CP230 is positive in chronic neutrophilic leukemia
DP230 has an indolent course
Show Answer & Explanation ▾
✓ Correct Answer: A. P190 has an indolent course

CML can have 3 types of transcripts. P190–Poor prognosis Seen in ALL P210–Seen in CML P230–Seen in CNL good prognosis Reference: Robbins & Cotran Pathologic Basis of Diseases 10 th edition, chapter 13, pg 633

Sample MCQ #2

50-year-old male complained of fatigability persistent for one year. Mild anemia massive splenomegaly. Hb 10 gm%, TLC of 1.2 lakhs, platelets 5 lakhs/ul, Neutrophils 60, Basophil 4, Eosinophil 6, Lymph 15, Myeloblast 3. Rest were myelocyte and bands. M:E ratio of 18:1, Most specific further investigation:

AFISH or PCR
BImmunophenotyping
CWhole body PET CT
DLeucocyte alkaline phosphatase
Show Answer & Explanation ▾
✓ Correct Answer: A. FISH or PCR

FISH or PCR to document BCR-ABL translocation.

Sample MCQ #3

A 60-year-old male living in hilly area has Hb of 16 gm%, TC 21000/mL. DLC showed metamyelocytes and myelocytes 40%, N25% L40%, E5%. Platelet count 3.25 lakh/u. He presented with hypertension and on examination, spleen was just palpable below costal margin. What is the next step?

ABone marrow with reticulin stain
BJAK STAT mutation assessment
CPhiladelphia chromosome
DErythropoietin levels
Show Answer & Explanation ▾
✓ Correct Answer: D. Erythropoietin levels

In this case, the clinical features are suggestive of polycythemia. Since, he is from hilly area, it may be due to secondary causes. To rule out primary (low epo) vs secondary cause (high epo), erythropoietin must be the next logical step. Reference: Robbins & Cotran Pathologic Basis of Diseases 10 th edition, chapter 13, pg 624

Sample MCQ #4

A 53-year-old man has had increasing fatigue for the past 6 months. On physical examination he has a palpable spleen tip. Laboratory studies show a WBC count of 189,000/microliter. The peripheral blood smear shows many mature and immature myeloid cells present. Cytogenetic analysis of cells obtained via bone marrow aspiration reveals a t(9:22) translocation. This translocation leads to formation of a hybrid gene that greatly increases tyrosine kinase activity. Which of the following genes is most likely translocated to cause these findings?

Ap53
BRb
Cc-abl
DNF-1
Show Answer & Explanation ▾
✓ Correct Answer: C. c-abl

The above history shows organomegaly with increased WBC counts. Peripheral smear with mature, immature myeloid cells and t(9:22) translocation is case of chronic myeloid leukemia. BCR-ABL fusion gene formation is an early, perhaps initiating, event that drives leukemogenesis. The crucial role of BCR-ABL in cancer has been confirmed by the dramatic clinical response of patients with chronic myeloid leukemia to BCR-ABL kinase inhibitors. Sample MCQ #5

A 3 year old child presents with scattered lesions in the skull. Biopsy revealed Langerhans giant cells. The most commonly associated marker with this condition-

ACD 1A
BCD 57
CCD 3
DCD 68
Show Answer & Explanation ▾
✓ Correct Answer: A. CD 1A

Langerhans cell proliferations take on different clinical forms, but all are believed to be variations of the same basic disorder. The proliferating Langerhans cells express MHC class II antigens, CD1a, and langerin. Langerin is a transmembrane protein found in Birbeck granules, cytoplasmic penta laminar rodlike tubular structures that in electron micrographs have a characteristic periodicity and sometimes a dilated terminal end ('tennis racket' appearance). Under the light microscope, the proliferating Langerhans cells do not resemble their normal dendritic counterparts. Instead, they have abundant, often vacuolated cytoplasm and vesicular nuclei, an appearance more akin to that of tissue macrophages (called histiocytes by morphologists) hence the term Langerhans cell histiocytosis. Sample MCQ #6

A 24 - year - old woman who presented with aquagenic pruritic had a Hb level of 20g/dL . Her lab investigations are given below, Which of the following is not seen in this condition ? Hb - 19g/dl Hematocrit - 52% S. erythropoietin - decreased JAK2 mutation - positive

AACUTE MYELOID LEUKEMIA
BBUDD - CHIARI SYNDROME
CMYELOFIBROSIS
DHEPATIC FAILURE
Show Answer & Explanation ▾
✓ Correct Answer: D. HEPATIC FAILURE

The given clinical scenario is suggestive of polycythemia vera. It is a myeloproliferative neoplasm, presents with history of hepatic vein thrombosis (Budd Chiari syndrome ) and can progress to Myelofibrosis. It can also undergo clonal evolution to AML. Hepatic failure is not seen in this condition. Polycythemia vera is strongly associated with activating point mutations in the tyrosine kinase JAK2. In this condition, clonal erythrocytosis is taking place autonomously i.e, independent of erythropoietin. Clinical features of polycythemia vera : Headache Hypertension Dizziness

Sample MCQ #7

In Langerhans Cell Histiocytosis, the characteristic abnormality seen is

ABirbecks granules
BMacrophages
CPlasma cell
DGiant cell
Show Answer & Explanation ▾
✓ Correct Answer: A. Birbecks granules

Birbeck granules Also known as Langerhans bodies or X granules Seen on Electron microscopy Rod-shaped organelles with a central striation and terminal vesicular dilation, giving them a 'tennis racket' appearance. Reference: Robbins 9th/pg 531

Sample MCQ #8

Delayed dentition is seen in all/except ?

ADown syndrome
BCongenital hypothyroidism
CRickets
DAll of above
Show Answer & Explanation ▾
✓ Correct Answer: D. All of above

Ans. is 'd' i.e., All of above Causes of delay in tooth eruption : 1) Impacted teeth 4) Cong hypothyroidsm 2) Down syndrome 5) Gaucher Cleidocranial dysplasia 6) Osteo petrosis
