About Vascular Heart Diseases MCQs

Vascular Heart Diseases MCQs span bicuspid aortic valve and small ventricular septal defects found on routine examination, through mitral stenosis with atrial fibrillation demanding warfarin, to endocarditis linked with colorectal tumours, in high-yield NEET PG revision.

Sample Vascular Heart Diseases MCQs with answers

Sample MCQ #1

Which of the following is not a described association for Mitral valve prolapse?

ANoonan syndrome
BKlinefelter syndrome
CMarfan syndrome
DBarlow syndrome
Show Answer & Explanation ▾
✓ Correct Answer: A. Noonan syndrome

Syndromes with MVP as a feature Marfan syndrome Ehler Danlos syndrome Klinefelter syndrome Osteogenesis imperfecta Autosomal dominant Polycystic kidney disease Pseudoxanthoma elasticum Barlow syndrome is synonymous with Mitral valve prolapse Noonan syndrome is associated with peripheral pulmonary artery stenosis, hypertrophic cardiomyopathy and atrial septal defect. Sample MCQ #2

A 65-year-old man has renal bruits, renal impairment and hypertension. Which one of the following tests is most appropriate?

AIntravenous urography
BRenal DTPA nuclear scan
CRenal DMSA nuclear scan
DMagnetic resonance angiography(MRA) of renal arteries
Show Answer & Explanation ▾
✓ Correct Answer: D. Magnetic resonance angiography(MRA) of renal arteries

From the options above, For renal artery stenosis, MRA remains the best investigation with high sensitivities and specificities. MRI is not effective for detecting stent thrombosis. CT angiography provides excellent imaging of the vessels and is less affected by stents. Risk of contrast nephropathy in renal impairment. Though Intra Arterial angiography remains Gold standard for diagnosis of Large vessel diseases. Doppler is very less expensive but is operator dependent and not useful for distal renal arteries and Fibromuscular dysplasia. Captopril renography has strong negative predictive value. Sample MCQ #3

A 34-year-old man presented for a routine check-up from company. He was symptom free, but clinical examination suggested a small ventricular septal defect. Which one of the following findings was most likely to have been present?

AAn early diastolic murmur
BA short systolic murmur at the right sternal edge
CA systolic murmur maximal at the apex
DA systolic murmur with thrill at the left sternal edge
Show Answer & Explanation ▾
✓ Correct Answer: D. A systolic murmur with thrill at the left sternal edge

Typically, a small VSD generates a pansystolic murmur at the left sternal border(LSB) accompanied by a thrill. The murmur may be heard at the apex but is usually loudest at the LSB . Very small defects may generate an early/late systolic murmur. Reference: Harrison's Principles of Internal Principles, 20 th edition, ch:234,264 pg:1673,1833

Sample MCQ #4

A 30-year-old heroine addict was found dead in his room. He was last seen alive by his neighbours around 4 weeks ago and he was presumed to be unwell over the past 4 weeks. At autopsy, his kidneys had a flea bitten appearance and renal biopsy showed sub epithelial and sub endothelial deposits of Immunoglobulin and C3. What could be the possible cause of these findings?

ASub acute Bacterial endocarditis.
BSLE
CRenal Tuberculosis
DDiabetes mellitus
Show Answer & Explanation ▾
✓ Correct Answer: A. Sub acute Bacterial endocarditis.

The patient is a IV drug abuser, who was presumed to be not well for the past few weeks. IV drug abuse increases the risk of Infective endocarditis. So the patient probably died of an untreated Infective endocarditis. The appearance of kidneys in IE is 'Flea Bitten' and microscopy shows immune complex deposition. It is more common in Sub Acute Bacterial endocarditis than Acute, since acute will be more symptomatic and the patient would have got treated. SLE presents with varying degrees of proliferation. Moreover, the history of drug abuse cant be ignored. Reference: Harrison's Principles of Internal Medicine, 21 st ed, Pg: 2337.

Sample MCQ #5

Which of the following cardiac conditions is not associated with continuous murmur?

ARuptured sinus of valsalva
BMR with AS
CPatent ductus arteriosus
DAnomalous left coronary artery
Show Answer & Explanation ▾
✓ Correct Answer: B. MR with AS

Causes of continuous murmur These murmurs persist throughout systole and diastole . Causes include : Patent ductus arteriosus Coronary arteriovenous fistula Rupture sinus of Valsalva aneurysm Aortopulmonary window ASD with MS Anomalous left coronary artery Reference : Harrison's Principles of Internal Medicine 21 st edition Page 2012

Sample MCQ #6

A 39-year-old healthy woman has a faint systolic murmur on physical examination. An echocardiogram is performed, and she is found to have a bicuspid aortic valve. In explaining the meaning of this finding to her, the most appropriate statement is that?

AAn aortic valve replacement is eventually likely to be required.
BOther family members are not likely to have the same condition
CShe should be treated with a cholesterol-lowering agent
DDown syndrome is a common association of this condition
Show Answer & Explanation ▾
✓ Correct Answer: A. An aortic valve replacement is eventually likely to be required.

Bicuspid aortic valve is perhaps the most common form of congenital heart disease in adults (0.5-1.4% of population). Bicuspid aortic valve (BAV) tends to occur sporadically although it can also be inherited in an autosomal dominant pattern. The prevalence of BAV disease among first-degree relatives of an affected individual is ~10%. Complications that can occur in BAV include aortic stenosis, aortic regurgitation and increased risk of aortic dissection. Aortic aneurysm may also be coexistent with BAV. Bicuspid valves have a propensity to wear out and calcify with aging . So an aortic valve replacement is eventually likely to be required in the 5th and 6th decades of life. BAV is seen in association with turner's syndrome . Reference: Harrison's Principles of Internal Principles, 20 th edition, ch:256 pg:1803,1804,1807

Sample MCQ #7

A patient presented with an episode of TIA. On examination he was found to have an irregularly irregular pulse. A 2d echo showed he had severe mitral stenosis. ECG showed atrial fibrillation. Which of the following is true regarding prevention of stroke. A. Aspirin is the only drug needed B. Warfarin is indicated C. NOACs are not indicated D. Patient with MS can be recommended for a mitral valve surgery

AA is correct
BA and D are correct
CB, C and D are correct
DAll choices are correct
Show Answer & Explanation ▾
✓ Correct Answer: C. B, C and D are correct

Warfarin is the anticoagulant of choice in valvular AF, For patients with AF or atrial flutter and an elevated CHA2DS2-VASc score of 2 or greater in men or 3 or greater in women, anticoagulation with warfarin (INR 2.0-3.0), is recommended.

Sample MCQ #8

A 72-year-old lady presented with dyspnoea and fever. She has a history of weight loss which has been investigated with colonoscopy which found a tumor of the sigmoid colon and she is awaiting surgery. On examination she has a systolic murmur and and ECHO shows vegetations on the mitral valve. A diagnosis of infective endocarditis is made. Which of the following organisms is associated with a high incidence of colorectal tumors?

ACampylobacter jejuni
BEnterococcus faecalis
CEscherichia coli
DStreptococcus bovis
Show Answer & Explanation ▾
✓ Correct Answer: D. Streptococcus bovis

Up to half of patients presenting with Streptococcus bovis endocarditis have colorectal tumors. Reference: Harrison's Principles of Internal Medicine, 21 st ed, Ch: 81 Pg: 638
