Sample Sleep Apnea MCQs with answers

Sample MCQ #1

A 36-year-old man visits his doctor because his wife has long complained of his snoring, but recently observed that his breathing stops for a couple of minutes at a time while he is sleeping. He undergoes polysomnography and ventilatory response testing to ascertain the extent and cause of his sleep apnea. The activity of the central chemoreceptors is stimulated by which of the following?

AAn increase in the PCO2 of blood flowing through the brain
BA decrease in the PO2 of blood flowing through the brain
CAn increase in the pH of the CSF
DA decrease in the metabolic rate of the surrounding brain tissue
Show Answer & Explanation ▾
✓ Correct Answer: A. An increase in the PCO2 of blood flowing through the brain

The central chemoreceptors, located at or near the ventral surface of the medulla, are stimulated to increase ventilation by a decrease in the pH of their extracellular fluid (ECF). The pH of this ECF is affected by the PCO2 of the blood supply to the medullary chemoreceptor area, as well as by the CO2 and lactic acid production of the surrounding brain tissue. The central chemoreceptors are not stimulated by decreases in PaO2 or blood oxygen content but rather can be depressed by long-term or severe decreases in oxygen supply. Sample MCQ #2

A 30-year-old male presents with hematuria and hemoptysis of two weeks duration. His urinalysis showed 2+ protein and dysmorphic RBC. His physician suspected a pulmonary renal syndrome and performs a renal biopsy. The pathologist uses a special stain to visualize the basement membrane. What is it?

ACongo Red
BMasons Trichrome
CPer-iodic Acid Schiff
DJone's Methanamine silver.
Show Answer & Explanation ▾
✓ Correct Answer: D. Jone's Methanamine silver.

Referring to the previous question, this patient is also suffering from a pulmonary renal syndrome. If the pathologist wants to stain the basement membrane, he might be suspecting Goodpasteur's syndrome. Jone's Methanamine silver is the stain used for basement membrane. Congo red is used for Amyloid. Masons Trichrome for collagen PAS for staining carbohydrate moiety in the cell membranes. Sample MCQ #3

Most common symptom of the Pertussis is

ASleep disturbing cough
BProlonged cough
CParoxysmal cough
DWhooping cough
Show Answer & Explanation ▾
✓ Correct Answer: A. Sleep disturbing cough

Sample MCQ #4

A 50-year-old woman who is morbidly obese presents with a plasma creatinine of 2.8 mg/dl.Which form of disease is likely?

ARenal tubular acidosis
BFocal segmental glomerulosclerosis
CNephritic syndrome
DMembranous glomerulonephritis
Show Answer & Explanation ▾
✓ Correct Answer: B. Focal segmental glomerulosclerosis

As with MPGN, FSGS is not a specific disease entity, it is a morphology of renal injury in several conditions that presents as nephrotic syndrome. The important ones are: Adaptive response to loss of renal mass as post nephrectomy, in obesity and in hyperfiltration. Hep B, Parvo virus, HIV Reflux nephropathy Sickle nephropathy Analgesics and heroine Hereditary podocytopathies: NPHS mutation, WT1 mutation, Fabrys disease, N acetyl neuraminidase deficiency, PLCE mutation, INF2 mutation, TRPC mutation, ACTN mutation. Sample MCQ #5

The correct description of Bruns garland syndrome is:

ADiabetic neuropathic cachexia
BLumbosacral radiculoplexopathy
CCranial mononeuropathy
DDiabetic cardiac autonomic neuropathy
Show Answer & Explanation ▾
✓ Correct Answer: B. Lumbosacral radiculoplexopathy

Bruns garland syndrome is also known as diabetic amyotrophy/lumbosacral radiculoplexopathy Asymmetrical, painful weakness involving quadriceps, iliopsoas and adductors of thigh; spares the hamstring muscles Most common form of Diabetic neuropathy is the distal symmetric sensorimotor peripheral neuropathy Most common cranial nerve involvement in Diabetes mellitus is Oculomotor nerve followed by abducens and facial nerves. Sample MCQ #6

A 39-year-old woman gives an eight month history of bloody diarrhoea. On average she has six bowel motions per day with associated urgency. She also reports weight loss of approximately 3 kg. On examination she is dehydrated and tachycardic. There is evidence of pallor and abdominal examination reveals left-sided tenderness. Which of the following pathological features would you expect to find given the likely underlying diagnosis?

ADiffuse mucosal inflammation
BSub serosal lymphoid aggregates
CNormal crypt architecture
DPresence of goblet cells
Show Answer & Explanation ▾
✓ Correct Answer: A. Diffuse mucosal inflammation

Chronic bloody diarrhoea in a young patient is very suggestive of a diagnosis of ulcerative colitis (UC) . The time course is too long for an infective cause and whilst Crohn's disease (CD) may cause a colitis, this is less frequent. CD often presents watery diarrhoea. The predominant left sided symptoms (urgency and anatomical distribution of the tendency) should also point towards UC. The answer here therefore is diffuse mucosal inflammation . All the remaining options are histological features of Crohn's disease . Transmural inflammation is seen in CD. Sub serosal lymphoid aggregates are seen in CD Normal crypt architecture is found in CD while crypt abscesses and goblet cell depletion are seen in UC.
