Sample Physiology of Hearing and Tuning Fork Tests MCQs with answers
Sample MCQ #1
The main function of middle ear is
AConduction of sound energy to inner ear
BAmplification of sound energy
CImpedance matching
DPitch enhancement of sound energy
Show Answer & Explanation ▾
✓ Correct Answer: C. Impedance matching
The middle ear converts sound of greater amplitude but lesser force, to that oflesser amplitude but greater force. This function of the middle ear is called impedance matching mechanism or the transformer action. This is to counterbalance the loss of sound energy in the inner ear fluids. Sample MCQ #2
A 40 year old patient developed sudden hearing loss on right side within a span of 12 hours. On audiometry, the right ear has 70dB hearing loss over 500, 1000 and 2000 Hz. All of the below are proposed management options in this patient except
AOral steroids
BCaroverine
CHyperbaric oxygen therapy
DCarbogen inhalation
Show Answer & Explanation ▾
✓ Correct Answer: B. Caroverine
This is a case of Sudden SNHL. Treatment(Proposed) : Bed rest Steroid therapy. Inhalation of carbogen(5% CO 2 + 95% O 2 ) Vasodilator drugs Hyperbaric oxygen therapy Caroverine is not used in the treatment of sudden SNHL. Sample MCQ #3
A patient presents to the OPD with complaints of bilateral hearing loss with worsened right ear hearing loss. On examination, the doctor diagnoses otosclerosis and recommends stapedectomy as the final resort to the problem. What would be findings of tuning fork tests based on which doctor recommends stapedectomy:
ARinne's test - positive: Weber's test - lateralized to left ear
BRinne's test - positive: Weber's test - lateralized to right ear
CRinne's test - negative: Weber's test - lateralized to left ear
DRinne's test - negative: Weber's test - lateralized to right ear
Show Answer & Explanation ▾
✓ Correct Answer: D. Rinne's test - negative: Weber's test - lateralized to right ear
Hearing loss is a presenting symptom in otosclerosis , usually starts in early twenties, often bilateral conductive type. In conductive type of hearing loss, Rinne's test shows negative result for frequencies (BC>AC) and Weber's test is lateralized to affected (worse) ear. Tuning fork test will show negative Rinne (BC>AC) first for 256 and then 512 Hz and after complete stapes fixation, for 1026 Hz. Weber will be lateralized to ear with greater conductive hearing loss. Absolute bone conduction may be normal.
Sample MCQ #4
One of the following is not the criteria to label a hearing loss as Sudden SNHL
AHearing loss should be more than 30 dB
BHearing loss should be seen on minimum 3 continuous frequencies in audiometry
CHearing loss should have occurred within 3 days
DBilateral Hearing loss should be there
Show Answer & Explanation ▾
✓ Correct Answer: D. Bilateral Hearing loss should be there
SUDDEN SNHL Definition- It is defined as sudden hearing loss of minimum 30dB It should be visible in at least 3 continuous frequencies in PTA It should have happened within 3 days. It’s unilateral. And has Unknown etiology. Hypothesis- Inflammatory cochleitis. Viral cochleitis. Autoimmune disease. Vascular cause. Treatment – DOC-oral steroids. Other treatments- Transtympanic injection of dexamethasone (using MICROWICK). Hyperbaric oxygen therapy. Betahistine (It is a labyrinthine vasodilator). Antiviral drugs. Sample MCQ #5
A steel factory worker is suffering from noise induced hearing loss. Which of the following is most likely to be affected?
AInner hair cells in Helicotrema
BOuter hair cells in basal turn of cochlea
CInner hair cells in basal turn of cochlea
DOuter hair cells in apex of cochlea
Show Answer & Explanation ▾
✓ Correct Answer: B. Outer hair cells in basal turn of cochlea
Noise Induced Hearing Loss causes damage to hair cells, starting in the basal turn of cochlea. Outer hair cells are affected before the inner hair cells. Outer hair cells are vulnerable to sounds and drugs while the inner hair cells are more resistant. Sample MCQ #6
All are the proposed treatment modalities for sudden sensorineural hearing loss except
AIntratympanic dexamethasone
BIntratympanic Mitomycicn C
CHyperbaric Oxygen Therapy
DMethylprednisolone
Show Answer & Explanation ▾
✓ Correct Answer: B. Intratympanic Mitomycicn C
Drugs such as cisplatin, mitomycin-C, 5-fluorouracil, pacititaxel, docetaxel and hydroxyurea have been used in concomitant chemoradiation. They act as radiosensitisers and potentiate the effect of radiation by their additive or synergistic effect. Reference: Diseases of Ear, Nose, AND Throat by PL Dhingra, Pg no. 426
Sample MCQ #7
A Patients presents to your clinic for evaluation of defective hearing. Rinnie's test shows air conduction greater than the bone conduction on both sides with Weber test lateralized to right ear. What is the next logical step?
AThis is indicative of Normal hearing
BSchwabach test
CRepeat Rinnie's test on right side
DGelle's test
Show Answer & Explanation ▾
✓ Correct Answer: B. Schwabach test
Schwabach and ABC(Absolute Bone Conduction) test both are used to test the bone conduction(functioning of cochlea). To give proper meaning to Rinne and Weber test findings, they should be followed by either of these. In ABC, external auditory canal is occluded while in Schwabach's test, EAC is not occluded. Sample MCQ #8
Patient has the complaint of right sided hearing loss and left ear hearing is normal. Tuning fork tests show that Rinne is Negative on right side and positive on left side. Weber test is lateralised towards left ear. What is the interpretation?
ARight moderate conductive hearing loss
BRight severe sensorineural hearing loss
CNormal hearing on both sides
DRight mixed hearing loss
Show Answer & Explanation ▾
✓ Correct Answer: B. Right severe sensorineural hearing loss
It is a situation of false negative Rinne. Rinne is negative in right ear but Weber is heard in left ear(better ear). This contradiction is the best hint for false negative Rinne which is seen in unilateral severe SNHL.
