About Acute Lymphocytic Leukemia (ALL) MCQs

Acute Lymphocytic Leukemia (ALL) MCQs distinguish lymphoblasts by TdT, CD1a and CD3 in T-ALL and PAX-5 in B-lineage disease, use PAS block positivity, and separate mediastinal ALL from thymoma. This is high-yield NEET PG haematology for FMGE, INI-CET and NEET SS.

Sample Acute Lymphocytic Leukemia (ALL) MCQs with answers

Sample MCQ #1

How will you differentiate mediastinal mass from thymoma differentiating it from ALL?

ACytokeratin
BCD1a
CCD3
DTdt
Show Answer & Explanation ▾
✓ Correct Answer: A. Cytokeratin

A mediastenal mass has a differential of thymoma and T cell ALL. T –ALL will have the markers TDT (Lymphoblast), CD 1a and CD3. Thymoma has two components: epithelial and lymphoid component. So is positive for EMA, cytokeratin 7 and 20, CD57 CD5, bcl-2, calretinin, vimentin, CD3, CD1a, CD20, CD99 and Ki67 and TdT. Sample MCQ #2

A 12-year-old boy is brought to the emergency room by his family. Over the past two weeks, he has developed marked fatigue, and his parents have noted pinpoint hemorrhages in his eyelids. On examination, the patient appears ill. His vital signs are temperature of 99.6°F, blood pressure of 100/65 mm Hg, and pulse rate of 102 bpm. The white blood cell count is 87,000 cells/μL, with 92% blasts, hemoglobin of 8.6 g/dL, and platelet count of 68,000 cells/ μL. Of the following, which would genetic testing most likely reveal?

At(15;17)
Bt(9;22)
Ct(12;21)
Dt(11:14)
Show Answer & Explanation ▾
✓ Correct Answer: C. t(12;21)

The patient has acute lymphoblastic leukemia. The symptoms and signs are consistent with an infiltrative process in the bone marrow, and the differential indicates an acute leukemia. In children, the majority (around 75%) of the acute leukemias are acute lymphoblastic leukemia, so, although other forms of leukemia are possible, the most common would be ALL. t(12;21), which translocates the TEL1 and AML1 genes, is characteristic of acute lymphoblastic leukemia. t(9;22) can be found in ALL, but most commonly in adult ALL. Of the translocations, t(11;14) is found in mantle cell lymphoma, t(14;18) in follicular lymphoma, and t(15;17) in acute myeloid leukemia. Sample MCQ #3

All of the following are seen in the development of T lymphocyte at a point; except:

ATdt
BCD34
CPAX5
DCd1a
Show Answer & Explanation ▾
✓ Correct Answer: C. PAX5

PAX-5 is a B Cell marker Reference: Robbins & Cotran Pathologic Basis of Diseases 10 th edition, chapter 13, pg 594

Sample MCQ #4

Which type of leukemia shows block positivity on PAS (periodic acid schiff) stain?

AChronic Lymphocytic Leukemia
BAcute Myeloid Leukemia
CChronic Myeloid Leukemia
DAcute Lymphocytic Leukemia
Show Answer & Explanation ▾
✓ Correct Answer: D. Acute Lymphocytic Leukemia

Acute lymphoblastic leukemia/lymphomas (ALLs) are neoplasms composed of immature B (pre-B) or T (pre-T) cells, which are referred to as lymphoblasts. Histochemical stains are also helpful, in that (in contrast to myeloblasts) lymphoblasts are myeloperoxidase-negative and often contain periodic acid-Schiff–positive glycogen granules.
