Sample Pelvic Inflammatory Disease MCQs with answers

Sample MCQ #1

A 16-year-old boy is being treated with desmopressin for diabetes insipidus. His plasma glucose level (fasting) is 108 mg/dl, sodium 139 mmol/l (137-144), potassium 4.5 mmol/l (3.5-4.9) and calcium 10.9 mg/dl. He still has complaints of polyuria, polydipsia and nocturia. What could be the most probable cause?

ADiabetes mellitus
BNephrogenic diabetes insipidus
CCentral diabetes insipidus
DSIADH
Show Answer & Explanation ▾
✓ Correct Answer: B. Nephrogenic diabetes insipidus

This boy most probably has nephrogenic diabetes insipidus due to hypercalcemia. In this condition, renal tubules are resistant to antidiuretic hormone (ADH), which is due to a receptor defect of vasopressin-2 receptor or a post-receptor defect in an ADH-sensitive water channel called aquaporin-2. Central DI (ADH deficiency) show good response to desmopressin (V2 agonist) administration. The findings do not suggest diabetes mellitus, which is also associated with these symptoms. In patients with the syndrome of inappropriate secretion of ADH (SIADH), low sodium levels are the hallmark.
