About Evaluation of Jaundice MCQs

Evaluation of Jaundice MCQs train serology reading in acute hepatitis B, cholestatic enzyme patterns, Courvoisier sign pointing below the cystic duct, and drug-induced cholestasis culprits — high-yield diagnostic reasoning for NEET PG, FMGE and INI-CET.

Sample Evaluation of Jaundice MCQs with answers

Sample MCQ #1

A 34 year old patient has had a hepatitis test for investigation of jaundice. He has a positive Hep BsAg (HBsAg) and has IgM antibodies to Hep B Core (anti-HBc). What is he likely to have?

AHepatitis D infection
BSeroconversion illness
CChronic hepatitis B infection
DAcute hepatitis B infection
Show Answer & Explanation ▾
✓ Correct Answer: D. Acute hepatitis B infection

The positive anti-HBc (IgM) and HBsAg suggests acute infection. When the infection resolves, HBsAg becomes negative and anti-HBc (IgG) is positive. In patients who have been vaccinated, HBsAg is negative and anti-HBs is positive. Reference : Harrison's Principles of Internal Medicine 21 st ed, Ch: 339, Pg: 3577

Sample MCQ #2

Which of the following enzyme elevation is/are less specific for the cholestasis jaundice?

A5 Nucleotidase
BGGT
CALP
DA and C both
Show Answer & Explanation ▾
✓ Correct Answer: B. GGT

ENZYMES THAT REFLECT CHOLESTASIS The activities of three enzymes-alkaline phosphatase, 5′-nucleotidase, and γ-glutamyl transpeptidase (GGT)-are usually elevated in cholestasis. Alkaline phosphatase and 5′-nucleotidase are found in or near the bile canalicular membrane of hepatocytes, whereas GGT is located in the endoplasmic reticulum and in bile duct epithelial cells. Reflecting its more diffuse localization in the liver, GGT elevation in serum is less specific for cholestasis than are elevations of alkaline phosphatase or 5′- nucleotidase. Some have advocated the use of GGT to identify patients with occult alcohol use.

Sample MCQ #3

A 47 year old man has painless jaundice. He has pale stools and dark urine. On examination he is deeply jaundiced and has scratch marks all over his body. On palpation of the abdomen Courvoisier's sign is positive. Where is the level of biliary obstruction?

ABile canaliculi
BIntrahepatic ducts
CHepatic duct
DCommon bile duct
Show Answer & Explanation ▾
✓ Correct Answer: D. Common bile duct

Courvoisier's sign is a palpable distended gallbladder on examination of the abdomen. Obstruction below the level of the cystic duct causes this, probably due to carcinoma of the pancreas. Reference : Harrison's Principles of Internal Medicine Ch:83, Pg: 659,

Sample MCQ #4

A 33 year old man presents with jaundice. He has been on a drug which was prescribed by his GP for several weeks. His blood tests show a bilirubin of 2.6 mg/dl, ALP of 450 U/l and AST 50 U/l. Which one of the following drugs is likely to cause cholestatic jaundice?

AChlorpromazine
BParacetamol
CIbuprofen
DAllopurinol
Show Answer & Explanation ▾
✓ Correct Answer: A. Chlorpromazine

Drugs causing Cholestasis: anabolic steroids containing an alkyl or ethinyl group at carbon 17 azathioprine cetirizine cyclosporine Chlorpromazine Co amoxiclav Erythromycin estolate diclofenac estrogen febuxostat indinavir mercaptopurine methyltestosterone tamoxifen ticlopidine. Reference : Harrison's Principles of Internal Medicine , 21 st Ed, Ch: 340 Pg: 2587

Sample MCQ #5

A 38 year old presents with a 10-day history of loose stools and mild abdominal discomfort, and recent onset of blood in stool. Ulcerative colitis was suspected by the gastroenterologist. Which one of the following is the investigation of choice?

ASigmoidoscopy
BColonoscopy
CBarium enema
DRadionuclide scintigraphy
Show Answer & Explanation ▾
✓ Correct Answer: B. Colonoscopy

Colonoscopy is the investigation of choice to evaluate ulcerative colitis (UC). Colonoscopy in UC shows continous involvement of mucosa with no skip lesions. Features such as loss of haustral pattern, erythema, oedema, granular mucosa, blood, pseudopolyps, erosion, ulceration are seen. Reference: Harrison's Principles of Internal Medicine, 21 st Ed, Ch:326 Pg: 2475

Sample MCQ #6

A 72 year old woman with longstanding hip osteoarthritis presents complaints of constipation and loose stool. She undergoes a sigmoidoscopy. A rectal biopsy shows pigment-laden macrophages in the lamina propria. What is the most likely cause?

ANon-steroidal anti-inflammatory drugs
BCrohn's disease
CDiverticular disease
DLaxative abuse
Show Answer & Explanation ▾
✓ Correct Answer: D. Laxative abuse

The pigment laden macrophages suggest that there is melanosis coli. The most common association is with laxatives. To a lesser extent, melanosis coli is also associated with ulcerative colitis. Reference: Harrison's Principles of Internal Medicine, 21 st Ed, Ch: 46 Pg: 307

Sample MCQ #7

A 65-year-old man presents with a five day history of lower abdominal pain and diarrhea. He has a history of chronic obstructive airways disease (COAD) and has had numerous acute infective exacerbations over the last three months. On examination he was dehydrated, with a temperature of 38.6°C, a blood pressure of 102/72 mmHg and has a distended, tender abdomen.Which of the following is the most appropriate investigation for this patient?

AChest x ray
BPlain abdominal x ray
CSigmoidoscopy and biopsy
DStool microscopy
Show Answer & Explanation ▾
✓ Correct Answer: B. Plain abdominal x ray

This is pseudomembranous colitis due to Clostridium difficile secondary to antibiotic usage for his COPD. Plain AXR is useful for diagnosing toxic dilatation and would be the investigation of choice here due to his abdominal distension, so toxic dilatation should be excluded prior to sigmoidoscopy. However it does not establish the diagnosis.Stool microscopy has no value but stool toxin assay is useful. A patient with diarrhea normally has involvement of the distal colon and rectum, and sigmoidoscopy with biopsy is helpful for rapid diagnosis but should not be performed if toxic dilatation is suspected.Patients with involvement of right colon usually have little or no diarrhoea. Reference : Harrison's Principles of Internal Medicine, 21 st ed, Ch: 138, Pg: 1069
